Services
All of our services are provided at low or no cost to registered families living in our service area. As of July 1st, we will operate on a sliding scale basis. Click here for the sliding scale form. If you do not live in Southwest Riverside County, you may still access our services, but on a fee-for-service basis. In order to access ANY services at the center, you must do the following (click on each link to download the required forms):
- Register your child(ren) as a client (s)
- Complete an income verification form
- Complete a liability waiver
- Submit proof of income (must be a copy of your last 1040-front page only)
- Provide a $20 deposit. (This is refundable on the day of the appointment or it will be applied to your payment). Click here for charge authorization.
To obtain the forms, click on each of the links above. Your paperwork must be completed and received by the center before an appointment can be scheduled. To have a packet mailed to you, please call the center. Forms may be FAXED to 951-587-9067 or mailed in.
It is very important that you arrive on-time to your appointment. We have a very full calendar every day. Each appointment is only scheduled for an hour.
